APPLICATION REQUIREMENTS
Grades 6–11
Applicants must be eleven (11) to seventeen (17) years old.
The applicant must have a quarterly general average of at least 80% with no grade lower than 78% in any academic subjects and in deportment.
Applicants with grades below the minimum requirements are welcome to apply. However, the application is subject to the approval of the Admissions Committee.
For Filipino Applicants
A scanned (PDF) copy of the Birth Certificate authenticated by the Philippine Statistics Authority (PSA)
A scanned (PDF) copy of a recent photo, passport size (4.5 cm. x 3.5 cm. or 1.8 in. x 1.4 in.)
A scanned (PDF) copy Report Card (SF9) (if applicable)
Additional Requirements for Grades 6–11 Applicants
A scanned (PDF) copy of the current school’s Government Recognition
An accomplished Recommendation Form from the Principal (school-to-school transaction)
An accomplished Recommendation Form from the Guidance Officer (school-to-school transaction)
For Foreign Applicants
(In addition to the above requirements)
Original School Records (Report Card and Transcript of Record) authenticated by the Consular’s Office in the country of origin (This is a requirement of the Department of Education for the issuance of the Learner Reference Number or LRN.)
A scanned (PDF) copy of the Birth Certificate (with English translation as applicable)
A scanned (PDF) copy of the Passport with Biodata page and Visa Status
A scanned (PDF) copy of the Alien Certificate of Registration (ACR) I-Card issued by the Bureau of Immigration
Documentary Requirements for Special Study Permit—
For Applicants with Dual Citizenship

A scanned (PDF) copy of a valid Philippine Passport
Certificate of Recognition as a Filipino Citizen or Identification Certificate as a Filipino
