• Enrollment/Assessment Procedure

    NEW STUDENTS APPLICATION PROCEDURES:
    
    CASA to GRADE 1
    STEP 1: Fill-up the Application Form using the On-line Application under the Admission tab or you may download the Application Form
                if you prefer to file it manually.
    
    STEP 2: Submit the accomplished form (for manual application) and the following requirements to the Registrar's Office.
    
           1. Clear copy of Birth Certificate from National Statistics Office (NSO) or Philippine Statistics Office (PSA)
           2. Two (2) recent 2x2 colored ID pictures (white background, scanned photos are not allowed) 
           3. Medical Certificate stating that the child is "fit to go to school"
           4. Certified True Copy of Report Card with Learner's Reference Number (LRN)
                  - Previous Grade level
                  - Current Grade level (latest grading period)
    
    STEP 3: Pay the non-refundable Application/Assessment Fee of P500.00 at the Cashier's Office.
    STEP 4: Get the Interview Slip indicating the date and time of interview.
    
    
    Note: 
           1. Only applicants who have complied and submitted the admission requirements shall be processed.
           2. Casa & Grade 1 applicants are only required to undergo an assessment/interview.  Assessment results 
               are released on the same day of the Interview/Assessment.
           3. Applicants for Grade 1 must show proof of completion of Kinder from a DepEd accredited school with 
               the corresponding Learner's Reference Number (LRN).
    
    
    GRADE 2 to GRADE 11
    
    STEP 1: Fill-up the following forms:
    
           1. Application Form using the On-line Application under the Admission tab or you may download the Application Form
               if you prefer to file it manually.
           2. Recommendation Form - Principal (for evaluation) 
           3. Recommendation Form - Guidance 
    
    STEP 2: Submit the accomplished forms and the following requirements to the Registrar's Office for screening and evaluation.
    
           1. Clear copy of Birth Certificate from National Statistics Office (NSO) or Philippine Statistics Office (PSA)
           2. Two (2) recent 2x2 colored ID pictures (white background, scanned photos are not allowed) 
           3. Medical Certificate stating that the child is "fit to go to school"
           4. Certified True Copy of Report Card with Learner's Reference Number (LRN)
                  - Previous Grade level
                  - Current Grade level (latest grading period)
           5. Signed Recommendation Form from the Principal
           6. Signed Recommendation Form from the Guidance Counselor
    
    STEP 3: Pay the non-refundable Application/Assessment Fee of P500.00 at the Cashier's Office.
    STEP 4: Get the Examination Slip indicating the date and time of exam.
    
    Additional Requirements For Foreign Students:
    
           1. Original Birth Certificate & School Records (Transcript, Report Card) with English translation duly 
               authenticated by the Philippine Foreign Service Establishment at the student's country of origin or legal residence
           2. Original and Photocopy of Passport
           3. ACR I-Card
           4. Documentary Requirements for Special Study Permit (SSP) 
           5. Certification of Recognition as a Filipino if with dual citizenship
    
    Testing Guidelines (Grade 2 to Grade 11 applicants)
    
           1. Be at the designated area at least 15 minutes before the test. Applicants arriving beyond 15 minutes 
               shall be re-scheduled on the next available slot.
           2. Present the Test Permit to the examiner.
           3. Bring the following:
                  - Sharpened Mongol pencils #2 and eraser
                  - School ID
                  - snacks and drinks
           4. Exam results are released through the Registrar's Office within three (3) working day process.
    STEP 1: REGISTRAR'S OFFICE
    
         a. Get Priority Number.
         b. Submit all registration /enrollment requirements.
              > Original Report Card
              > Transcript of Records
              > ACR I-Card / Special Study Permit (Foreign students)
              > Certification of Recognition as a Filipino if with dual Citizenship
    
         c. Get assessment and sign the Enrolment Conforme
         d. Receive OBMC Information Kit
    
    
    STEP 2: CASHIER'S OFFICE
    
         a. Get Priority Number.
         b. Pay tuition and other fees.
         c. Get official receipt.
    
    STEP 3: ID ROOM
    
         Station 1: SOPTAF & Fetcher's ID
    
              > Submit accomplished SOPTAF form and have the parent's photo taken. 
              > Submit the signed Fetcher's form and have the authorized fetcher's photo taken. 
    
         Station 2: Student's ID
    
              > Present the official receipt and have the student's ID photo taken. 
    
    STEP 4: BOOKSTORE
    
         a. Get the assessment and pay for the materials and uniform. 
         b. Get official receipt.
         c. Pick-up materials according to schedule.
    
    STEP 5: ID ROOM
    
         > Claim the SOPTAF, Fetcher's and student ID.
